Physicals must be dated ON OR AFTER June 1, 2021. You must be registered on Family ID. Completed (sections 6 & 7) physicals can be uploaded directly to Family ID or turned into the CB West Athletic Office.FAMILY ID IS NOW OPEN! All Athletes must be registered on Family ID to participate!FAMILY PASS INFOMY PAYMENTS PLUS is now open to purchase your athletic pass. The cost is $60 for family passes (immediate family only), $70 for two cards, $30 for student pass only.Reminder that students 8th grade and under must be accompanied to games by a parent.Passes must be presented at games.NO LIST WILL BE AVAILABLE.PICTURES OF PASSES ON CELL PHONES WILL NOT BE ACCEPTED.*PIAA Physical Form (Sections 6 - 7) – will need to be completed and taken to your physician for signature.
