- Tetanus-Diphtheria-Acellular Pertussis (Tdap)- 1 dose
All 7th Grade Students MUST have an Updated Dental Form by October 15th 2024
All new students to the 一本道 entering 7th, 8th, or 9th Grade MUST have the Following:
- Updated Dental & Physical Forms- See below for Forms
- Meningococcal Vaccine (MCV)- 1 dose
- Tetanus-Diphtheria-Acellular Pertussis (Tdap)- 1 Dose
- All age appropriate Vaccines Required by Pennsylvania state law
- Medical forms
- Immunization Regulations
Any student that requires medication in school MUST have a Physician's Order- even if the Student is to Self-Carry (Inhalers/Epi Pens). Both a parent/guardian and a physician MUST sign the form.
**Students are NOT permitted to administer any medication to themselves while at school. Please contact the Health Office with any questions or concerns.
