    Please Note: This form is used as a placement guide for AM and PM kindergarten requests.  It is not a guarantee for your request.  If your request is work related, submit a letter(s) from your employer(s) verifying your daily work schedule and the need for the placement.  Requests supported by a letter from both parents' employers will receive priority, in the order of receipt.
